The first step is to stop using synthetic fertilizers and pesticides ASAP!!! They are awful for your family and pets.
I am in Long Island and I have been using this landscaper that uses only organic lawn care services. You definitely need to follow the plan year-round to see results, but I am telling you it is worth it.
It can be a little expansive if you want to have everything done professionally, but there are many things you can do yourself.
