Curious why bathroom fans must vent through the roof while you are considering not venting the RMH through the roof.
You will need the chimney wherever it is to be two feet higher than any roof within ten feet; If the roof has much slope that could be a tall exposed structure which will probably need to be braced to the roof. If you can exit with the chimney very near the roof peak, you will have the best possible draft with the least exposed chimney structure. A penetration near the top of the roof will have the least potential runoff to have to divert, so lowest risk.
A 45 degree wall exit would be good for airflow, but would direct water down the exterior slope right against the wall, requiring the same level of waterproofing as a roof penetration.
